- Login as Admin
- Click on Organization Management
- Navigate to the organization you need to add this form to
- Click on Templates
- There will be columns for user types, users, and locations.
- User Types allows you to assign it to all providers and/or all patients
- Users allows you to assign it to specific individuals
- Locations assign it to everyone (provider ad patient) in that location
Upon user registration, the form will be automatically presented to the corresponding users for completion.
"Required for Intake User Completion" means that a patient will not be allowed to check into a session until they have completed this form.
